If you live in Slovakia

Apply in person or through an authorized representative at any office of the public prosecution or any municipality with a Registry Office.

If you live outside of Slovakia

Apply at your nearest Slovakian embassy or consulate.

You will need to provide an application form and the original identity card.

If you are not a citizen of Slovakia, you must also present your original passport and a certified translation of your birth certificate.
